2012 New London Softball Apparel Order Form (pdf)

Friendly Reminders:

- All orders and payment are due to Fox Cities Embroidery by Friday, April 20, 2012.

- Orders received after the April 20th deadline will be charged a $25.00 late fee.

- All orders are the responsibility of each family.  Orders are to be placed, paid for and picked up directly with / from Fox Cities Embroidery in Hortonville.

- If you would like to request an earlier pick up for embroidery items, you must submit a separate order form containing JUST embroidery items.

- Due to the nature of screen printing, those items will not be available for early request pick up.
